The Bute Community Nursing Team is looking for a dynamic individual who is passionate about patient care, an influencer and possesses proven leadership experience along with excellent communication skills.
The Community Nurse Team Lead/ SCN post will have professional and managerial responsibility of the Community nursing team on Bute.
The post holder will ensure the highest standard of personalised nursing care is delivered to patients and their families in partnership with the Community Hospital, A&E, GP Practice, Adult Social Work and all members of the multi-disciplinary team on the Island.
The Bute Extended Community Care Team is a busy team providing a wide variety of clinical care to patients in their own homes, as well as daily dressings clinics within the community hospital.
The Team work closely with our OT and Physiotherapy colleagues to promote reablement and to provide a seamless service to our island community.
