Job description: Sanctuary Personnel is currently looking for an experienced Band 6 Senior Community Mental Health Practitioner (CPN) to take on a new ongoing contract role.
Location: Stowmarket
Pay Rate: £24.58 – £25.58 per hour (assignment rate)
Contract Type: Locum, 3-month contract
Shift Pattern: Full time
Duties:
- Support with the review and management of the waiting list in the South Community Mental Health Team
- Case review, telephone calls and face to face reviews with service users experiencing complex mental health challenges within a secondary mental health service
- Completion of documentation, CPA/NCPA reviews, section 117 reviews, ward rounds, attendance at MDT meetings and ensuring safety planning, along with safe discharge from services
Requirements of the Senior Community Mental Health Practitioner:
- Registration as a qualified Nurse, Social Worker or Occupational Therapy
- Sound risk assessment and clinical decision making skills
- Assessment skills relating to complex mental health presentations
- The ability to confidently administer depot medication
- Knowledge of the CPA process
- Experience of working within Adult Community Mental Health Teams is desirable
