The project has been set up to provide Mental health sites in each of the 12 neighbourhoods across LLR (Leicester, Leicestershire, and Rutland). These sites are to have 1 worker in, within the capacity of a Mental health community reablement worker using the 3 conversations approach.This advert is for 2 full time equivalent workers, community reablement workers, working within Northwest Leicester neighbourhood or Oadby & Wigston Neighbourhood. These roles will potentially be working with people with complex mental health so it is desirable that you have, and can demonstrate, experience with the presentation of this.About the RoleAs a Mental Health Community Reablement Worker, you will deliver a community-based enablement and inclusion service, which supports people to learn or relearn skills, reduces social isolation, and encourages people to become part of their community and as independent as possible. You will work with individuals and the key people in their lives to identify specific needs and wants which you will then discuss with the individual, setting out how these will be achieved and including strategies that can be used to manage any identified risks.As part of your role, you will support individuals to explore a range of community options such as access to employment, voluntary opportunities, recreational/social pursuits, and community, cultural or spiritual activities. Where necessary, you will provide information to commissioning workers about an individual’s eligible needs as well as identifying and reporting potential safeguarding, mental capacity, and other high-risk situations where a person’s safety or liberty may be at risk.As part of this role, you will be working in the 3 Conversations approach. You will connect with individuals to find out their wants and needs to support them in their reablement journey. You will step away from the old ways of working and assessment-based approach and will use conversations to discuss the support that may be needed. You will work using the 3 conversations principles and golden rules and connect the individuals to support rather than hand them over.   • Hold a relevant health/social care qualification to NVQ Level 2 or equivalent or have equivalent relevant experience.
  • Have relevant experience of working with adults with mental health conditions and physical and learning disabilities in a social care setting and of working with other agencies.
  • Have an understanding of recovery, social inclusion, self-directed support, and person-centred planning as well as knowledge of a variety of conditions and how these can impact people’s lives as well as a firm understanding of the 3 Conversations approach.
  • Have a good understanding of the services that are available from external parties such as the NHS and community agencies and an awareness of how social care organisations fit within the wider community/voluntary sector.
  • Have knowledge of risk assessment and be able to work with individuals to develop strategies to manage any identified risks and have experience with working with those who have complex/long term mental ill health conditions.
  • Have good communication, prioritisation, time management, and IT skills.
  • Be able to travel to various locations throughout the county in a cost-effective manner.
