Job description: Band 5 Nursing AssociateLocation: BristolContract Type: PermanentHours: Full time and part time shifts availableSalary: £28,407-£34,581 per annum, dependent on experienceWe are proud to be working with Sirona and their NHS@Home service, who are currently seeking a Band 5 Nursing Associate to join their team based in Bristol.Do you want to be part of a new, emerging service who are passionate about preventing hospital admissions, supporting early hospital discharges and delivering personalised care in the comfort of people’s homes?In keeping with the new developing transformation change to using virtual ward models, Sirona Care & Health are collaborating with UBHW & NBT NHS Trusts to deliver the NHS@Home service to patients across BNSSG (Bristol, North Somerset & South Gloucestershire).As a team we are blending medical innovation with bespoke care, bridging the gap between hospital and home. A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology and home visits. The innovative approach enables us to deliver safe, high quality and personalised care to people at home where they would rather be.We know that people recover faster and better in their own environments and our dedicated teams of trained staff endeavour to support this. A hybrid model of virtual, telephone and face to face consultations can be delivered as required, allowing patients to recover in their own home with continued clinical support.What will you be doing?Working in the NHS@Home team, you’ll contribute to a multi-disciplinary approach, undertaking board rounds and working alongside consultants, advanced care practitioners, nurses and therapists to devise effective treatment plans that make a real difference to the lives of local people.Your duties will include:· Providing acute care to patients with complex and specialist needs in the community, across Bristol, South Gloucestershire and North Somerset· Working alongside the multidisciplinary team to develop, implement and evaluate care plans· Assessing the health and psychological needs of the patient within the community setting· Planning and implementing treatment and use a wide range of services to meet their needs· Utilising a wide range of clinical skills to support the ongoing management of patients in their home environment e.g. delivering IV therapies, venepuncture, cannulation and wound care· Communicating effectively, networking and liaising to ensure high quality responsive, safe, efficient and compassionate care is provided for patients, carers and familiesWhat do you need?· Degree level qualification in nursing· Current NMC registration· Full driving licence and access to car in work time· Experience of working in a health care or social care setting providing direct patient care, demonstrating a clear understanding of how health can impact on all aspects of everyday life· &
