Job description: An exciting opportunity has arisen for a Registered Nurse to join our Community District Nursing Team.

The Role

The post holder will be a Registered Nurse who will support the community nursing service in the delivery of the highest quality of health care and will be responsible and accountable for providing skilled nursing interventions as appropriate. The post holder will ensure a person-centred approach to care delivery which is safe and effective to ensure and maintain quality strategy.

The post holder will be expected to provide an accessible, appropriate, high quality and effective care service to all age groups in the community, by working in partnership with patients, care providers and other agencies. You will be responsible and accountable for assessing, planning and implementing of programmes of care. These will be developed in partnership with patients, family, carers and significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health. You will also contribute to the development of local policies and guidelines and promote effective health improvement in a community setting

What we’ll need you to bring

Registered Nurse with valid NMC Registration

The post holder will be required to demonstrate excellent team working skills with the ability to work using own initiative

A full UK Driving licence is essential for this post

Effective listening, communication and interpersonal skills

Ability to work with people and as part of a multidisciplinary team

Location and Working Pattern

  • This role will be based in District Nursing within the Houldsworth Centre
  • The working pattern for this role will cover various shifts over a seven day period
