About the Post:

This is an exciting opportunity to join a highly ambitious, innovative, energetic, friendly, and high achieving 0-19 Public Health Nursing Service we are seeking to recruit a Specialist Community Public Health Nurse (Health Visitor or School Nurse) who is passionate about wanting to lead and support the provision of a high-quality Children in Care service. To ensure continued provision of the Public Health Nursing Service within Barnsley Metropolitan Borough Council we are seeking to recruit a dedicated, motivated, and innovative Specialist Community Public Health Nurse to work in our Children in Care Team.

About the post

The Children in Care SCPHN will work as an integral part of a multi-agency service to provide a seamless pathway for Children in Care and Care Leavers. The primary focus of this role will be to undertake statutory health assessments, develop and undertake age-appropriate health promotion advice, attendance at statutory reviews, and the delivery of the healthy child programme with children and young people in care to improve their life chances and health outcomes. The post holder will work with children in care, unaccompanied asylum-seeking children/young people, children with complex health needs, care leavers and their parents/carers. Following national and local guidelines, plan, organise, deliver, and evaluate comprehensive health assessments to assess and formulate the complex needs of a looked after child or young person using evidence-based knowledge to develop health plans towards improving the long-term health outcomes for looked after children and will act as a resource providing health advice to children, carers, and professionals.

In the role as SCPHN, you will need,

You will need to be a registered Specialist Community Public Health Nurse (SCPHN) with sound knowledge and understanding of the Healthy Child Programme and related policies and pathways. You will need to be able to identify risk, assess, plan, and deliver care, providing evidenced based interventions to support children, young people, and care leavers within Barnsley. Be a highly motivated, innovative, and adaptable practitioner who is committed to taking a child centred approach with the ability to identify health needs as early as possibility and improve health and wellbeing by promoting health, preventing ill health, and reducing inequalities. You will need to be able to plan and prioritise your own workload and the ability to travel to a range of venues and settings is essential. We are seeking an experienced nurse who can contribute to ongoing service improvement initiatives demonstrating a commitment that the “voice” and lived experience of the child and young person is at the heart of all decision making and clinical care planning. Experience of safeguarding and multi-agency working is also key to the role. The team also take responsibility for children placed out of borough. This includes undertaking the health assessment and/or liaison with other agencies and services, coordinating effective care and acting as an advocate where necessary.
