Job description: Sanctuary Personnel is currently seeking an experienced Band 6 Community Learning Disabilities Nurse to take on a new locum opportunity to work in Halton.
Pay Rate: £30 – £31 per hour (assignment rate)
Location: Halton
Contract Type: Locum, 3-month contract
Duties:
- Health action plans/health questionnaires/health passports
- Support with learning disability health checks/ referrals to services
- Health facilitation
- Health promotion work – healthy eating/lifestyle
- Mens/womens groups – sexual health/relationships/personal hygiene/breast and testicular awareness
- Bespoke training
- Training– to enable a wide range of staff, including those who work in social services, hospitals and the independent sector, to become more familiar with how to support people with learning disabilities to have their health needs met
- Sleep hygiene
- Sexual health and relationship work
- Psychiatry clinic /mental health support
- Preventing admission to hospital by providing community based treatment working alongside the Community learning disability team and intensive support team as part of transforming care
- Supporting people with complex and enduring needs within the community (in partnership with the LD Community Matron)
- Support with stomp pathway/medication monitoring
- Support with activities of daily living
- Behavioural support/assessments/plans
- Lead professionals for clients on Dynamic support database
- Walk in the park every Monday
- Link workers GP surgeries/data cleanse project alongside ICB
- Assessment of learning disability
- Desensitisation
- Transition work
- Weekly liaison with local Hospital safeguarding teams
- Reasonable adjustments
- End of life care/support
- MCA/DOLS assessments
- Positive behaviour support plans
Requirements of the Community Learning Disabilities Nurse:
- Degree or equivalent RMN/RNLD
- Experience working as a Learning Disabilities Nurse
- Current NMC Registration
