Care Coordinator Pay Rate: £30 per hour (assignment rate)
Location: Oxford
Contract Type: 3-month contract, likely to be extended

Shift Pattern:
- Full time but part time applicants will also be considered
- Monday-Friday 9am-5pm (would consider 4 longer days)
Duties:
- Care of older adults with mental health needs both functional and dementia
- Assessment of mental health needs, care planning with psychosocial interventions and medication
- Care coordination
Requirements of the Care Coordinator:
- Degree or equivalent in Mental Health Nursing
- Community experience, experience in older adult mental health is preferred but experienced clinicians with an interest in older adults will be considered
- NMC Registration
- Experience working in an NHS setting
- Car driver essential with access to own vehicle
