Care Coordinator Pay Rate: £30 per hour (assignment rate)

Location: Oxford

Contract Type: 3-month contract, likely to be extended

Shift Pattern:

  • Full time but part time applicants will also be considered
  • Monday-Friday 9am-5pm (would consider 4 longer days)

Duties:

  • Care of older adults with mental health needs both functional and dementia
  • Assessment of mental health needs, care planning with psychosocial interventions and medication
  • Care coordination

Requirements of the Care Coordinator:

  • Degree or equivalent in Mental Health Nursing
  • Community experience, experience in older adult mental health is preferred but experienced clinicians with an interest in older adults will be considered
  • NMC Registration
  • Experience working in an NHS setting
  • Car driver essential with access to own vehicle
