Overview:
At Compass Associates, we’re excited to collaborate with a distinguished Private Healthcare company that employs a distinctive approach to delivering exceptional care to patients with Complex Care needs, such as Acquired Brain Injury, Spinal Cord Damage, Tracheostomy, and Ventilator Care within the community.
Together, we are actively headhunting a dynamic Nurse Case Manager with a RGN background to support Adult Complex Care packages across North London, including Hounslow and surrounding areas. In this role, you’ll ensure Adult Complex Care patients receive the utmost care by providing clinical guidance and support to HCAs within your packages.
Reporting to the regional lead nurse and being accountable to the director of nursing, you’ll work closely with the wider Multidisciplinary Team (MDT), including Care Coordinators. Your responsibilities will include overseeing HCAs within your package, ensuring the provision of safe care aligned with NMC and CQC requirements and individual care plans, such as supporting the signing off of HCA competencies related to Tracheostomy, Ventilator, or NG Feeding care.
As a Nurse Case Manager, you’ll play a pivotal role in our team, offering face-to-face support to clients and their families. Your duties will involve crafting personalized care plans, collaborating closely with a dedicated team of caregivers, and providing clinical training and support.
Hybrid:
This role involves a blend of remote work and on-site visits to your packages in North London. Additionally, you’ll need to commute to the head office in Wimbledon at least once a week and attend nursing training at HQ once a month.
Requirements:
- Valid NMC registration under RGN
- Experience in complex care settings such as ICU, HDU, or A&E
- Minimum of 2 years’ post-qualification experience
- Experience with Tracheostomy, Ventilator, PEG Feeds, and NG Feeding
- Ability to clinically support HCAs and sign off on their competencies.
- Hold a valid UK driving license.
Location:
This is a hybrid role where you’ll manage your own schedule, working from home and across your packages in Hounslow and nearby areas. Your typical shift pattern will be 9:30 AM to 5:30 PM, with flexibility to start earlier or finish later if required by your packages.
