Location: Lymington
Contract Type: Ongoing contract
Pay Rate: £25 per hour (assignment rate)
Shift Pattern: Full Time – part time will be considered

Duties:
- Case management of patients at high-risk od repeat hospital admissions
- Identify the needs of patients and the correct services to liaise with
- Provide care in or as close to the patient’s own home as possible
- Provide an expertise in their own discipline to the wider team
Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy
- Recent UK experience working as an Occupational Therapist
- Recent NHS experience
- Registered with the HCPC
- Able to work in an MDT
- Car driver (preferred)
