Vacancies:

Older People Team – Hertsmere

A rare opportunity has arisen in the Hertsmere locality team for a Community Care Officer. We are looking for an enthusiastic person who is passionate about promoting the wellbeing of older people and carers.

You need to have good communication skills, the ability to plan your time effectively, managing competing demands and prioritising your workload. You will need to understand the principles and core values of adult social care, following Hertfordshire’s Connected Lives Principles. You will be working alongside other agencies and supporting partnership working, attending multi-disciplinary meetings and working autonomously.

The team works in a hybrid model which supports flexible working and a work/life balance.

Hospital

  • Princess Alexandra Hospital Team- Harlow
  • Lister Integrated Discharge Team – Stevenage
  • Community Hospital Team – East & North Herts

You will work as part of a busy social care team at Princess Alexandra Hospital or you will work as part of a busy integrated hospital discharge team at Lister Hospital, completing social care assessments to ensure that people are discharged in a safe and timely manner. You will develop close working relationships with the hospital’s Discharge Team and care providers to ensure effective care planning. You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support.

Older People Team – Hemel Hempstead

This is an exciting opportunity to join a high performing and supportive team working to support citizens over 65 years who live in a vibrant, multi-cultural town. We are committed to working in collaboration with our key partner agencies (health, social care and mental health services). The aim of this role to promote independence by assessing older people to identify their desired outcomes to maximise independent living whenever possible.

You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being. You will keep the service user at the centre of all activity and address assessed needs within the legislative framework. You will need to be able to work autonomously within agreed timeframes and manage frequently changing situations. You will work as part of an integrated team, attend multi-disciplinary meetings and need to have excellent communication skills.

Hertsmere: Herts Valleys Older People & Sensory Services Team

Driving Roles Only: As part of this role, you will be required to travel to other HCC Sites/Residences either using your own mode of transport or a company car. In light of this, the ability to drive and/or travel around the County is required for this role a DVLA check will be required on appointment.

English Fluency: The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required)
