We have a great opportunity for you to join our Alliance team as our Community Navigator working in and around the Durham area, supporting our customers living with mental health conditions to access and engage with services and organisations, whilst promoting positive mental health and wellbeing. Our team is responsible for representing all the alliance members which include 7 providers, a commissioner and 7 subcontractors.

Typical day as a job title

  • Follow up on referrals and connecting customers to relevant services.
  • Person-centred planning for each customer.
  • Be the link that keeps everyone connected.
  • Help change our customer’s lives through engagement with their carer’s, and professionals involved in their support network.

You bring

  • Great organisational skills, with a calm approach to tasks.
  • Be a good communicator using the ‘make every contact count’ approach (Mecc).
  • You’ve worked in a care environment – maybe you’ve cared for a child/parent etc.
  • It’s important that you have a little experience with technology, but we’ll support you with this.
  • To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage.

Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to external candidates for this role. This remains under regular review.

You’ll work alongside our fabulous team who are super passionate about delivering for our customers & communities. To us we’re not just a team we’re a ‘work family.’ There are 5 of us in the team our manager Julie has worked for Home Group for over 20 years and most recently as the alliance manager “so is perfectly placed to support you as you join us”. Julie has a real soft spot for her gorgeous cockapoo Daisy and loves nothing more than going for long walks with her when she’s not at work.

Job details

  • Working Monday to Friday 9 am to 5 pm.
  • Our main base being in Waddington Street Centre in Durham.
  • Our service works with customers for 4-6 sessions making sure they are connected to the relevant services they need.
  • The alliance covers the whole of County Durham, the community navigator will have a dedicated patch/area to cover within the County.
  • You’ll need an Enhanced DBS check done and we pay for that.
