Adult Social Worker – Community Discharge to Access Team
Company Name: Warrington Council
Location: Warrington, Cheshire
Pay Rate: £30 per hour
Hybrid Schedule: Office day is Monday, with flexible remote work options
Case Load: Low case load with 3 cases assigned per week
Travel Requirement: Role includes travel around Warrington, so access to a car with business insurance is a must
Experience Required: D2A (Discharge to Assess), Mental Capacity Assessments, and Adult Social Work experience are essential
Referral Bonus: Refer someone and receive £300 referral bonus
Key Responsibilities:
Assessing adults care and support needs
Developing and implementing support plans
Liaising with health professionals and other agencies
Ensuring timely and effective discharge from hospital
Key Requirements:
HCPC registration
Social Work Degree or equivalent
Experience in D2A and Mental Capacity Assessments
Access to a car with business insurance
Strong communication and interpersonal skills.
