In the Mid-Sussex community team, you will be working across the Mid-Sussex area from East Grinstead to Burgess Hill and surrounding areas, including from the A23 to the eastern edge of the West Sussex County Council area. The role involves carrying out the Local Authorities, Adult services responsibilities under the Care Act 2014. This involves undertaking strengths-based assessments in individuals own homes or within residential or nursing homes. As well as completing assessments you will be completing plans, implementing, monitoring, and reviewing care packages and reassessing individual’s needs. Although the community teams are busy, they love that no 2 days are the same and everyone is committed to improving lives of West Sussex residents.

About The Job

Salary: £27,334 to £29,777
Working pattern: 37 hours per week
Contract type: Permanent
Locations: Horsham office base but need to be able to travel to Mid-Sussex area to visit customers.

Our Assistant Care Managers have a supportive working environment and well-planned inductions with help from across the friendly and considerate team. There are approximately 14 members in the Mid-Sussex Community Social Work Team, with a mixture of qualified and unqualified team members, so whether it’s in person, on the phone or even in the chat group there is always someone on hand to offer help and support.

You will receive regular, supportive, and reflective supervision from senior social work colleagues, but you will also need to use your own initiative where appropriate. West Sussex is committed to positive work/ life balance, enabling flexible working balanced with the needs of the business. Following discussions there could be some requirement to work flexibly, out of office hours to assist customers, carers, and families. You will be required to travel throughout the Mid-Sussex locality area including locations that are not always easily accessible by public transport as can be very rural. Therefore, a driver’s licence is essential.

To be successful in this role you will need to use your initiative and be able to engage and build relationships with customers to provide a high level of customer services. An important part of the role is the need to complete and keep accurate records of all the amazing work you have undertaken. This will include producing reports and correspondence for a variety of audiences including people receiving the service, their families and carers, multi-disciplinary colleagues, and other agencies.

Experience and Skills

Key Skills (These will be used as the shortlisting criteria): * Ability to analyse and interpret information and prioritise needs in order to make recommendations/decisions which will have a positive outcome.

  • Ability to work independently and as part of a team, working under pressure and to tight deadlines.
  • Ability to work in a multi-agency environment to deliver a service to those people who access or need the service.
  • Ability to communicate complicated or sensitive information to a range of audiences to achieve positive outcomes; to be able to communicate clearly with customers reflecting the service objectives and standards.
  • Sound and accurate IT and keyboard skills.
  • Ability to recognise the limits of own accountability and responsibility, and to seek appropriate support, advice and supervision.

Qualifications and/or experience:

  • GCSE English and Maths or equivalent qualification or experience operating at this level of literacy and numeracy.
  • NVQ Level 3 in Care. Desirable
  • Experience of providing support/working with people in a Social Care environment.
  • Experience of forming effective working relationships within a multi-agency setting.
  • An awareness of the policies, practices and procedures relevant to Social Work. Experience of the assessment and review process of care packages.
  • Experience of applying relevant legislation, policies and procedures to individual pieces of work.
