Job description: West Berkshire Council is seeking a highly motivated and compassionate individual to join our Adult Social Care Service as a Care Practitioner. Reporting to the Resource Centre Manager at Phoenix Resource Centre offices, you will play a vital role in supporting the delivery of high-quality care to vulnerable adults in our community.
Key Responsibilities:
- Provide person-centred care and support to service users in accordance with their care plans and individual needs.
- Assist service users with personal care, medication administration, and daily living tasks.
- Promote independence and social inclusion by engaging service users in meaningful activities and promoting community involvement.
- Build strong relationships with service users and their families, ensuring their dignity, privacy, and wellbeing are maintained at all times.
- Maintain accurate and UpToDate records of care provided in accordance with relevant policies and procedures.
Skills and Attributes:
- Previous experience working in a care setting, providing direct support to vulnerable adults.
- A compassionate and caring nature with strong interpersonal skills.
- Ability to communicate effectively and sensitively with service users and their families.
- Basic understanding of health and safety procedures and the ability to follow care plans accurately.
- A commitment to continuous learning and professional development.
- NVQ or QCF qualifications in Health and Social Care would be advantageous.
