Job description: Are you passionate about making a positive difference in the lives of vulnerable adults within your community? West Berkshire Council’s Adult Social Care Service is seeking a dedicated Care Practitioner to join our supportive team. Reporting to the Support Outreach Worker, you will play a crucial role in providing essential care and support to individuals in need.

Key responsibilities:

  • Delivering high-quality care and support to vulnerable adults, promoting their independence and wellbeing
  • Assisting with personal care, medication administration, meal preparation, and other day-to-day activities
  • Building professional and supportive relationships with service users, their family members, and external agencies
  • Collaborating with the wider care team to develop, implement, and review care plans
  • Maintaining accurate and UpToDate records to ensure effective monitoring and evaluation of services provided

The ideal candidate will possess the following skills and attributes:

  • Experience in providing care and support to vulnerable adults, preferably within a social care setting
  • A compassionate and person-centred approach, with a genuine commitment to improving the lives of individuals
  • Excellent communication skills, both verbal and written, allowing for effective engagement with service users, colleagues, and external stakeholders
  • Strong organizational skills, with the ability to manage time effectively and prioritise workload
  • A relevant qualification in Health and Social Care or a related field would be advantageous
  • A flexible mindset and willingness to work as part of a dedicated team
