Job description: Team: COA – Young Adults and Independent LivingFamily: Supporting – CBG7Salary: £26,421- £28,770 per annum pro rataHours: 37 hours per weekLocation: You will be expected to attend a CBC office location as often as needed to carry out your role, which will be no less than once per month Contract Type: Hybrid – PermanentClosing Date: 21st April 2024Interview: 30th April 2024About the role:The Young Adults and Independent Living Team (YAaIL) supports vulnerable people who have a Learning Disability, an Autistic Spectrum Condition or a Physical Disability. The Community Assessment Officer will work with young people age 16+ in the Adults Team which is a countywide team.You will work to build strong relationships with the local community, other professionals and employers to make sure people have access to fulfilling lives and achieve their aspirations. Your role will include assessing, reviewing and support planning. Tenacious, proactive and approachable, you’re adept at building professional relationships and have dealt with a range of partners including Health, Housing and Education. You will have an understanding of the SEND reforms, The Care Act 2014 and will have care management skills. A positive, determined and diplomatic professional, you’ll know how to build links that count. You’ll work with partners to make sure young people get – and know about – great opportunities.The Community Assessment Officer will have responsibility for the delivery of statutory duties commensurate with supporting people within Adult Social Care. You will discharge these duties in a manner that sees customers at the centre of their care and support, along with, demonstrating behaviours that result in the delivery of compassionate, caring, responsive, and proportionate care and support that is of high quality and evidence best value.Succeed, and you will be part of a varied and dedicated team where flexibility and commitment go a long way. This is a care management role-Have some knowledge of The Care Act 2014 and able to assess and review in line with the legislation.-Have some knowledge of the Mental Capacity Act-Understand adult safeguarding process-Have some experience in assessing and commissioning services-Have experience working in an MDT way-We are a county wide service; the candidate needs to be able to drive and undertake face to face visitsSucceed, and you will be part of a varied and dedicated team where flexibility and commitment go a long way.
