Job description: The role of Coordinator is to oversee and lead a small team of support workers to ensure the people we support have their care and support provided to them in the way they choose.

About you…

  • Qualified to NVQ Level 3 or willing to work towards
  • Experienced in leading and supporting a team operationally.
  • Have a positive attitude to disability and the transforming care agenda (homes not hospitals for people with a learning disability or Autism)
  • Experience of Positive behavior support models
  • Good verbal and written communication skills
  • Able to prioritize own and others work, and motivate staff members to be the best they can be
  • Able to work flexibly as part of a rota to include weekends, bank holidays and evenings.

About the role…
Supported living is part of the wider Community Care services. The team provide care and support to adults with a learning disability within a small, supported living service. Other duties include:

  • To promote social inclusion, reducing isolation and contribute to service development to meet both individuals and service outcomes.
  • To maintain the staff rota to ensure adequate support is provided, coordinating annual leave, training and sickness cover amongst the team.
  • Arrange, coordinate and contribute to multi-disciplinary planning meetings for individuals to ensure an integrated coordinated health, social care and community approach to service delivery.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.

The Community Care Services Coordinator interviews will be held in March. Exact date to be confirmed.
