Social Work Adult Services in Falkirk is currently undergoing a wide-scale service transformation and as part of this the Care Home Assurance and Review Team has recently been made permanent. The team will focus on completing statutory reviews of outcomes of adults over 18 who are living permanently in a care home. It will also support quality assurance, monitoring in care homes and improvement of outcomes for adults who live in care homes.

We are looking for enthusiastic and motivated Social Workers /Occupational Therapists who are passionate about improving outcomes for people living in care homes.

You will adopt a person -centered approach that focuses on the adult and their carer/family members whilst undertaking outcome focused assessments and reviews. You will be expected to fulfil the role of delegated guardian for adults in care homes and review the powers accordingly. You will be familiar with Health and Social Care Standards: My Support, my Life and you will complete monitoring of these standards. You will support improvement and contribute to quality assurance in care homes You will participate in duty systems and rotas relevant to CHART and along with colleagues, you will be expected to contribute to the development of innovative ways of working to ensure that adults living in care homes outcomes are improved.

Experience of working in a community social care or health setting and knowledge of Community Care legislation is essential as is a strong commitment to multi -agency working.

You must be a Social Worker with a DipSW, CSS, or CQSW and registration with SSSC is essential or hold a relevant Occupational Therapy qualification and be registered with the HCPC .

You must have the ability to travel as per service demand.

The post is 37 hours per week but applications from those who wish to work part-time will also be considered.
