Job description: JustOT are looking to recruit a Community Home First Social Services Occupational Therapist for a locum position with our client in Dorset.The pay for this position is £35 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.We are looking for Occupational Therapist to Join Our Home First Team
Home First support hospital discharge and play a vital part in ensuring flow across the system. Work with acutes for complex early discharge planning, moving and handling, specialist equipment, Care Dorset reablement beds, RCR’s within community and joint work with Home First Team and peer forum. Reduces impact for localities of individuals leaving hospital with new or increased CA eligible needs.Early discharge support, complex cases, double handed care, long term conditions where continued support by LA identified in the community.
Work within hospital and community setting responding to demand across Dorset.6 month contract
