Job description: Sanctuary Personnel is currently looking for an experienced Band 6 Community Mental Health Nurse (CPN) to take on a new ongoing contract role.Location: London. Working across sites in South West London.Pay Rate: £24.87 – £35.00 per hour (assignment rate)Contract Type: Locum, 3-month contractShift Pattern: 37.5 hours per week (Monday – Friday, 09:00 am – 17:00 pm)Duties:
- Get to know all patients on the team’s caseload, and to review those on their personal caseload at the frequency agreed by the team
- Plan, implement and evaluate nursing interventions delivered in the community, and participating in multidisciplinary care plans
- Keep in contact with patients’ relatives, friends, carers and professionals in other agencies as appropriate
- Administrate, order and maintain a stock of medication and other medical sundries, in accordance with relevant policies and procedures
- Co-ordinate the care of those patients for whom the CPN is the nominated care co-ordinator
- Act as social supervisor for those patients for whom the CPN is the nominated social supervisor, including producing pro-forma reports for the Ministry of Justice (after receiving appropriate training, if necessary)
- Respond, sometimes at short notice, to requests to see the team’s patients either individually or jointly with other team members (e.g. when a patient is in the red zone)
- Attend and participate in all daily team zoning meetings unless absent by prior arrangement
- Attend CPA meetings or Tribunals where appropriate
Requirements of the Community Mental Health Nurse:
- Degree or equivalent in Mental Health Nursing
- NMC Registration
- Be contactable by mobile telephone or other agreed method at all times during working hours
- Communicate/liaise effectively within relevant agencies to ensure that an integrated programmed of care is provided throughout
- Be able to manage medication in accordance with the Policy and the NMC Standards for administration of drugs
- Demonstrable experience of working with people who experience serious ongoing mental health problems
- Experience of working in a multidisciplinary team
- Experience of assessing and managing crisis with those with a severe mental illness
- Evidence of continuing Professional Development
