Salary: £35,392 – £42,618 per annumJob Purpose:Sanctuary Personnel are seeking a Senior Mental Health Practitioner (Band 6) to join our client’s team working within the Access to Intervention service for adult mental health referrals on the Isle of Wight. The successful candidate, with a professional qualification, will have experience in crisis mental health assessments and interventions, serving individuals with mental health concerns. Responsibilities include triaging referrals, conducting assessments, providing brief interventions, and guiding service users towards suitable care pathways. Additionally, responding to crisis situations within the community and police stations when necessary is expected.The role entails shift work covering a 24/7 schedule, including public holidays, with a rotational pattern.Job Statement:

  • Actively participate in triaging, processing, and assessing referrals to the Access to Intervention service.
  • Conduct comprehensive mental health assessments and risk evaluations for referred individuals.
  • Plan and deliver acute and intensive crisis response care within the community.
  • Collaborate with service users and other agencies to ensure continuity of care post-service.
  • Contribute to service development and professional practice enhancement.
  • Maintain relationships and behaviour in accordance with Equalities Framework competencies.
  • Ensure all communications adhere to confidentiality standards and best clinical practices.

Key Tasks:Communication and Relationships:

  • Gather and process complex information from various disciplines efficiently.
  • Ensure clear communication of intervention plans to service users, families, and involved agencies.

Analysis and Judgement:

  • Plan and organise workload effectively, including shift coordination.
  • Ensure safe and timely responses to all referrals.

Planning and Organising:

  • Provide alternative services when appropriate, coordinating with other organisations.
  • Liaise with mental health services for optimal treatment pathways.

Patient and Client Care:

  • Conduct timely assessments of health and social needs, formulating appropriate care plans.
  • Ensure evidence-based interventions and risk management practices.
  • Collaborate with multidisciplinary teams to deliver holistic care.

Service and Policy Development:

  • Demonstrate innovative practice and contribute to service improvement.
  • Work in partnership with statutory and non-statutory organisations.
  • Participate in service practice review and development.

People Management and Development:

  • Supervise non-professionally qualified staff, including appraisals.
  • Participate in the Trust’s appraisal scheme and share knowledge with colleagues.

Information System Use and Management:

  • Effectively utilise electronic patient records and databases.
  • Ensure timely and accurate data entry for maintaining client records.

Involvement in Surveys and Research:

  • Actively engage in practice review, audit, and clinical governance initiatives.

Requirements:

  • RMN, Social Worker, Learning Disabilities Nurse, Psychological Wellbeing Practitioner or OT with mental health exp

Expected salary: £42618 per year
