The salary for this permanent Community Health Manager job is £67,355 – £80,716 per annum.

Duties:
- Manage and provide leadership for the development and provision of effective and efficient integrated Community based Health Services with responsibility for general and specialist services/ teams/ individuals
- Enable a high-quality person-centred service which provides direct support to service users with a range of needs in the community using evidence-based interventions within agreed care pathways
- Provide effective leadership and management to the integrated multi-professional team, with direct managerial responsibility for senior team members
- Ensure that person-centred assessment, planning, Implementation & evaluation of care meets professional and organisational standards
- Manage the team’s resources, budget and staff in compliance with the policies, procedures & guidelines and statutory responsibilities
- Be an integral member of the Community Health & Wellbeing senior leadership team, assisting in the strategic planning, development and delivery of services across the service area
- Ensure service user/carer involvement in the development of standards and services
- Facilitate service user choice and empowerment through promoting independence and the personalisation agenda
- Ensure services maintain consistency with best practice guidelines
Requirements of the Community Health Manager:
- Registered first level Nursing qualification
- Professional knowledge acquired through degree supplemented by specialist training or equivalent experience to Master’s level equivalent in a related field
- Teaching/mentoring qualification
- Full qualifications or to be achieved by second gateway
- Management or Business qualification
- Good communication skills
- Empathetic and caring nature
- Previous work in a similar team
- Basic keyboard skills
- Current and valid driving licence
- Ability to undertake audit, analyse results and draw conclusions
- Understanding of clinical governance and risk management
- Ability to co-ordinate analyse and summate advice from a range of agencies and professionals
- Experience of working in or managing multi-disciplinary teams
