Applications are invited from District Nurses and experienced band 5 Community Staff Nurses, if applying as a Band 5 there is an expectation that the individual will be able to show evidence of additional learning, advanced assessment and prescribing would be advantageous but not essential.

Applicant’s should be competent in patient-centred assessment, care planning and evaluation of nursing care for patients with long term conditions, tissue viability concerns and those receiving palliative care. Applicants should be able to demonstrate a wide range of clinical skills.

The applicant should have good leadership skills and be able to deputize for the team lead when required and support existing band 6 caseloads across the locality.

The post holder will be expected to work closely with service users, family and carers as well as the multi-disciplinary/ multi-agency team members in the community to support person centred care.

In order to apply for this position you must have a Live PIN with the NMC.UK as a first level registered nurse and possess a driving license.
