Pay rate – £38-39 per hour
Contract role – 6 months
CHC teamPertemps are recruiting for an experienced Adult’s Social Worker to work as an Advanced Practitioner. This will be to work within Continuing Healthcare.The role is to attend review DST’s with nurse assessors; find and review evidence to present at the DST; make DST recommendations on behalf of the LA; and work in partnership with the LA/ICB/NHS/providers/families.There is plenty of hybrid working available, along with Face to Face visits
