Salary Range: £38,092 – £45,389 pro-rata, per annum (inclusive of £3,000 per annum market supplement for Senior Social Workers)

Join our diverse and friendly Continuing Healthcare Team and work collaboratively with Health colleagues to support assessments for both NHS Continuing Healthcare (NHS CHC) and under the Care Act 2014, including assessments for those leaving hospital and in the community.

No two cases are the same and you’ll enjoy a diverse and manageable caseload, working across all client groups from younger adults, physical disabilities, mental health and older adults. Working within this complex and challenging area of health and social care law, you’ll have the opportunity to really stretch your social work skills and to make the most of personal development opportunities.

As a qualified Social Worker or Registered Nurse with UK based Social Work experience, you’ll ideally also have experience of working in NHS CHC. If you don’t have this background, you’ll need to have a strong desire and commitment to learn and develop your skills and knowledge in the NHS CHC arena. Inquisitive and with strong analytical and problem-solving skills, you’ll enjoy collaborating with colleagues to achieve successful outcomes within this dynamic multi-disciplinary environment.

Whether you are an experienced Social Worker or Nurse, looking to stretch yourself and take on a new challenge, or recently qualified looking to kickstart your career within NHS CHC, we can offer you a unique experience working alongside colleagues with a diverse range of skills, within a supportive space. Our newly qualified Social Workers are paid at £32,026 per annum, pro rata for the first year of employment.

Travel Requirements

You will be required to travel to various locations within Hampshire and it is essential that you have a current drivers’ licence and access to a reliable vehicle on a regular basis.
